Many experts believe that greater (and different) delivery system organization is

fundamental to improved quality and efficiency. This report summarizes and presents a

framework for understanding recent research literature linking delivery system organizational

attributes with health care efficiency and quality. The focus is on physician groups, as physicians

represent the core of any organized delivery system. The report also highlights areas for further

research and refinement of the framework and discusses whether and how policymakers can

promote specific physician group attributes as a means of improving value.

Do Primary Care Physicians Treating Minority Patients Report Problems Delivering High-Quality Care?

Abstract:
Do Primary Care Physicians Treating Minority Patients Report Problems Delivering High-Quality Care?

Primary care physicians who treat a disproportionate share of black and Latino patients provide more charity care, see more patients, depend more heavily on low-paying Medicaid, and earn lower incomes than physicians seeing mostly white patients, a new Commonwealth Fund-supported study finds.

As documented in the Health Affairs Web Exclusive article, Do Primary Care Physicians Treating Minority Patients Report Problems Delivering High-Quality Care?, such practice constraints can have a profound effect on the ability of physicians to deliver high-quality care to patients. For instance, physicians treating large numbers of minority patients typically spend less time with each patient and have a harder time obtaining specialty care referrals than do other physicians.

The researchers, James Reschovsky and Ann O'Malley from the Center for Studying Health System Change, say that expanding insurance coverage, raising Medicaid payments to equal Medicare payments, and increasing the resources available to physicians who serve low-income and minority populations could all go a long way toward reducing disparities in quality.

Full text is available at: http://content.healthaffairs.org/cgi/content/abstract/hlthaff.27.3.w222?ijkey=hPLueyDJn1sno&keytype=ref&siteid=healthaff

Conclusions

• The care of minority patients is concentrated in prac

Medicare Information for Pharmacists: March 17, 2008

Abstract:
CMS Releases the Final 2009 Call Letter…

CMS released the final 2009 Call Letter http://www.cms.hhs.gov/PrescriptionDrugCovContra/Downloads/CallLetter.pdf today, containing information on the Part C, cost-based, and Part D programs combined into one document. Anecdotal reports have indicated that small doses generally lead to euphoria and increased flow of energy while large doses often result in sedation. Although all three ingredients may contribute to these effects, most experts attribute the psychoactive effects to the alkaloids found in betel nuts Betel nut is reportedly used by a substantial portion of the world's population as a recreational drug due to its CNS stimulant activity.

Found originally in tropical southern Asia, betel nut has been introduced to the communities of east Africa, Madagascar, and the West Indies. There is little evidence to support clinical use of betel, but the constituents have demonstrated pharmacological actions. The main active component, the alkaloid arecoline, has potent cholinergic activity. Constituents of areca are potentially carcinogenic.

Long-term use has been associated with oral submucous fibrosis (OSF), pre-cancerous oral lesions and squamous cell carcinoma.

Acute effects of betel chewing include worsening of asthma, low blood pressure, and rapid heart beat.

Synonyms Amaska, areca nut, arecoline, arequier, betal, betelnusspalme, betel quid, chavica etal, gutkha, hmarg, maag, marg, mava, mawa, pan, paan, Palmaceae (family), pan masala, pan parag, pinang, pinlang, Piper betel Linn. Although osteoporosis is responsible for the majority of hip fractures, and accounts for more than eighteen billion dollars in health care expenditures annually, on average only 20% of patients are ever screened or treated for the disease. A new monograph developed by The Joint Commission through an unrestricted educational grant by NPC, is now available. The monograph contains 10 voluntary measures of care, designed for a variety of healthcare settings. "Improving and Measuring Osteoporosis Management" is intended for a professional audience and contains clinical information and practical tips for implementing an osteoporosis improvement initiative, whether clinical care is given in a hospital, emergency department, rehabilitation facility, home health agency, or doctor's office.

Johns Hopkins Center for Public Health Preparedness Online Training Modules

Abstract:
Johns Hopkins Center for Public Health Preparedness Online Training Modules
Follow the link below to access training modules on the following emergency preparedness and response topics: Agriculture, Food and Water; Chemical, Biological, Radiological, Nuclear Terror; Infectious Disease and Vaccines; Legal Issues; Mental Health Preparedness; and General Public Health Preparedness.
http://www.jhsph.edu/preparedness/training/online/index.html

National Center for Disaster Preparedness Online Training Center

Abstract:
National Center for Disaster Preparedness Online Training Center
These courses, offered by Columbia University's Mailman school of Public Health, cover topics including: Point of Distribution Medical Evaluation Officer Training; Basic Emergency Preparedness for Dental Professionals; and Dealing with Disaster Related Trauma in Young Children. Guidance materials outline three overarching strategic goals that states' plans should address: ensuring continuity of state government and agency operation, protecting citizens, and maintaining critical infrastructure and key assets. Several operating objectives are included in each goal. For example, for maintaining critical infrastructure, states are encouraged to build private-public partnerships and beef up protection and information sharing. According to the research team, led by Holly Mead, Ph.D., of George Washington University, U.S. minorities may disproportionately live in regions where the quality of care is suboptimal.

The chartbook is divided into the following sections:

The Demographics of America
Disparities in Health Status and Mortality
Disparities in Access to Health Care
Disparities in Health Insurance Coverage
Disparities in Quality
Strategies for Closing the Gap.

http://www.commonwealthfund.org/publications/publications_show.htm?doc_id=672908

HIPAA resources

Abstract:
What Physician Executives Need to Know about HIPAA (Health Insurance Portability and Accountability Act): See Link below

Office of Civil Rights- HIPAA home page: http://www.hhs.gov/ocr/hipaa/
Include Business Associates Agreement template, fact sheets, patient handouts about HIPAA in English and Spanish, HIPAA rules and regulations.

Links to Emergency Preparedness and HIPPA concerns: http://www.hhs.gov/ocr/hipaa/emergencyPPR.html
Includes:
EP for persons with disabilities
Disaster Response Tools
Cultural Competence in Disaster Mental Health Programs
much more

Our Baby's New Teeth: Spanish and English Resource Guide

Abstract: Our Baby's New Teeth: Spanish and English Resource Guide

Self Report of Skin Problems among Farmworkers in North Carolina

Abstract:
Migrant and seasonal farmworkers are exposed to a variety of irritants and allergens that can cause skin problems, including plants, biological agents, such as bacteria and fungi, and chemicals, such as pesticides, fertilizers, and petroleum products. This study estimates the prevalence of self-reported skin problems among Latino farmworkers and identifies associated risk factors.

Green tobacco sickness and skin conditions among migrant Latino farmworkers

Abstract:
Green tobacco sickness (GTS) affects approximately one-quarter of tobacco workers.
